Neonatal Sepsis

Neonatal Sepsis is one of the major reasons for neonatal mortality. Newborns are at a risk of sepsis due to low immunity Newborns can acquire early sepsis within 72 hours of their birth. Late sepsis can occur from more than 72 hours to 28 days of age.

The risk of neonatal sepsis rises considerably under the following circumstances

  • Preterm delivery
  • Premature rupture of membranes > 18 hours before birth (PROM > 18 hours)
  • Infection in amniotic fluids (chorioamnionitis)
  • Group B streptococcal colonisation during pregnancy
  • Mother suffering from fever around 2 weeks prior to delivery
  • Difficult delivery
  • Maternal urinary tract infection

Symptoms of neonatal sepsis:

The neonate suffering from sepsis usually show the following symptoms:

  • Lethargy
  • Refusal of feed
  • Respiratory distress
  • Decreased spontaneous movements
  • Seizures (meningitis)
  • Skin pustules (>10)
  • Low body temperature (hypothermia) or fever (hyperthermia)

Diagnosis of neonatal sepsis:

To identify neonatal sepsis the following tests are conducted by the doctors:

  • Complete blood count (CBC)
  • C-reactive protein (CRP)
  • Blood culture and sensitivity
  • Lumbar puncture
  • Chest X-ray for babies who have respiratory distress

Treatment of neonatal sepsis:

The course of treatment for sepsis is determined by the doctor depending on the following factors:

  • Neonate’s age, general condition, and maternal risk factors
  • The extent of the infection

Early treatment is key in case of sepsis. Treatment of neonatal sepsis includes intravenous antibiotics and other symptomatic treatment. Strict asepsis guidelines should be implemented in all of the Neonatal Intensive Care Units (NICU).

An early introduction of mother’s milk helps the neonate in improving immunity. It helps in fighting the infection and thus leading to reduced hospital stay and early discharge.
